Parent and Guardian Information

Co-Parent or Second Guardian Information
- First and last name, phone number, and email address
- Relationship to child
- Whether they are authorized for child pickup
Co-parent information is optional and only collected when a second guardian is applicable.
Child Information
- First and last name, gender, and date of birth
- Potty training status
- Primary care physician name and phone number
- Allergies, current medications, dietary restrictions, and special needs
- Additional medical notes
- Comfort and behavioral preferences (what helps your child feel comfortable in new settings, what helps when they need comfort)
Care and Scheduling Details
- Care type preference (full-time or drop-in)
- Drop-in frequency and days of the week needed
- Preferred start date
Emergency Contact Information
- Names, phone numbers, and relationship to child for two emergency contacts
Pickup Authorization Details
- Names, relationships, and phone numbers of individuals authorized to pick up your child
- Driver's license numbers for identity verification during pickup
- Custody agreements or pickup restrictions, where applicable
Consent and Communication Preferences
- Media release consent (photos and videos of your child)
- First aid authorization
- Sunscreen application authorization
- Agreement to Terms and Conditions
- Opt-in preferences for transactional and marketing text messages
Technical and Payment Information
- Payment information — billing details processed through our secure payment system
- Communication records — messages sent through our system, including text messages and emails
- Form submission data — browser type, page URL, time zone, and session identifiers collected during form submission for security and fraud prevention
How We Use Your Information
We use the information we collect to:
- Provide care for your child — enrollment processing, daily check-in and check-out, tracking allergies and medical needs, administering first aid or sunscreen with your permission, and contacting you or your emergency contacts when needed
- Verify pickup authorization — confirming the identity of anyone picking up your child using the authorized pickup list and driver's license information you provide
- Communicate with you — sending updates, reminders, schedule changes, and responding to your questions
- Process payments — billing for childcare services
- Improve our services — understanding how families use our programs so we can make them better
- Marketing — only with your explicit opt-in consent, we may send promotional messages about new programs, events, or special offers
- Comply with regulations — meeting Idaho childcare licensing requirements and maintaining required records
Text Messages (SMS/MMS)
If you provide your phone number and opt in to receive text messages from JellyBeans PlayCare Center, here is what to expect:
Transactional Messages
- Drop-off and pickup reminders, schedule confirmations, care updates, billing reminders, and emergency notifications
- Message frequency varies based on your child's care schedule and center activity
- Message and data rates may apply depending on your wireless plan
Marketing Messages
- Special offers, event announcements, program updates, and promotional content
- Sent only if you separately opt in to marketing messages — this consent is completely separate from transactional messages

Your Rights Regarding Text Messages
- Text message consent is entirely optional and is not required for enrollment
- You can opt out of text messages at any time by replying STOP to any message
- Reply HELP for assistance
- Opting out of marketing messages will not affect transactional messages related to your child's care, and vice versa
- We do not share your phone number or opt-in data with third parties for their marketing purposes
- Consent to receive text messages is not a condition of purchasing any goods or services
You can unsubscribe from marketing emails by clicking the "unsubscribe" link at the bottom of any promotional email.
Fraud Prevention (reCAPTCHA)
Our enrollment form uses Google reCAPTCHA Enterprise to protect against spam and automated abuse. When you submit a form, Google may collect hardware and software information (such as device and application data) and send it to Google for analysis. Your use of reCAPTCHA is subject to Google's Privacy Policy and Terms of Service.
How We Protect Your Information
We take your family's data seriously:
- Information is stored in secure, access-controlled systems
- Only authorized staff members can access your child's records
- Payment information is processed through encrypted, PCI-compliant systems — we do not store credit card numbers
- Physical records (if any) are kept in locked storage
- Driver's license numbers and custody documentation are treated as sensitive information with restricted access
Sharing Your Information
We do not sell, rent, or share your personal information with third parties for their marketing purposes. We may share information only in these situations:
- Service providers — companies that help us run our business (payment processing, communication tools, form and data management) under strict confidentiality agreements
- Legal requirements — if required by law, court order, or to protect the safety of children in our care

- Idaho licensing compliance — information required by state childcare licensing authorities during inspections or audits
Data Retention
We retain your family's information for as long as your child is enrolled and for a reasonable period afterward to comply with legal, accounting, and Idaho childcare record-keeping requirements. Specifically:
- Active enrollment records — kept throughout your child's time in our care
- Post-enrollment records — retained for up to three years after your child's last day, or as required by Idaho childcare licensing regulations
- Payment records — retained as required for tax and accounting purposes
- Communication records — text message and email logs retained for up to two years
You may request deletion of your personal data at any time (see Your Rights below). Some information may need to be retained to meet legal obligations even after a deletion request.
Your Rights
As a parent or guardian, you have the right to:
- Access your child's records and your personal information we have on file
- Correct any inaccurate information — please notify us promptly if your contact details, medical information, or authorized pickup lists change
- Request deletion of your personal data (subject to legal retention requirements outlined above)
- Withdraw consent for photos, videos, first aid, sunscreen application, or marketing communications at any time
- Update pickup authorizations — add or remove authorized pickup persons, update custody documentation, or change driver's license information
- Opt out of text messages at any time by replying STOP

Children's Privacy
We collect information about children only as necessary to provide childcare services and only with parent or guardian consent. We do not knowingly collect information from children directly through our website. All information about children in our care is collected from and managed through their parents or legal guardians.
